Use when reporting an incident or crime to SAPS, opening a case, or providing a witness statement.
How to edit this template
- 1Replace everything in orange square brackets with your own details, then delete the brackets.
- 2You can print this and hand it to the investigating officer, or use it to organise your account before making your statement at the police station.
- 3Stick to facts you personally witnessed or experienced. Use dates, times, and descriptions rather than assumptions or opinions.
- 4The statement must be signed in front of the officer taking it (they act as Commissioner of Oaths) — do not sign it in advance.
- 5Keep a copy of your statement and note down your case (CAS) number for your records.
- 6A filled-in example is included on the last page — use it as a guide for tone and length, not for copying word-for-word.
Before you submit it
- Confirm the recipient's latest format, document and submission requirements on its official website.
- Replace every example name, date, address and reference number with your own accurate information.
- Remove personal information that the recipient has not asked for, especially when sending the document by email.
- Check whether the final document must be signed, witnessed, commissioned or certified before submission.
